Reconnections Early Stage Program

Frequently Asked Questions

How do I know if my loved one is appropriate for Reconnections or the Day Center?

We will work with you to find the best fit for your loved one! If your loved one is still in the early stages, they could be appropriate for either program, and we can do an assessment for both Reconnections and the Day Center to ensure the best placement.

Here are some factors to consider:

Timing: Reconnections runs from 10:00 am to 3:00 pm. Our Adult Day Health Center is open Monday through Friday from 7:30 am to 5:30 pm.
Location: Reconnections is offered in Fairfax, Sterling, and Alexandria. The Adult Day Health Center is located in Fairfax.
Care Needs: Our Adult Day Health Center offers nursing care, medication management, and assistance with most all care needs. In our Reconnections program, participants must be able to handle care needs independently.
Wandering: Our Adult Day Health Center is a secure facility, and provides safe space for wandering and walking. Participants in our Reconnections program are able to come and go with staff support.

Can my loved one participate in both programs?

Participants often start in our Reconnections program first, and on average participate for over a year. Others may start in our Adult Day Health Center, or transition there from the Reconnections program. We will work with you to both determine which program is the best fit for your loved one, and also to determine when it might be time to transition from Reconnections to the Day Center. We want to always ensure that participants are engaged and appropriately challenged by their program to set them up for success.
