Join Us in the Day Center!
We’re so happy you’re interested in having your loved one join us. Our admissions process is simple and supportive, with four easy steps.
First, schedule a tour to meet our team and see the center. After your visit, complete the application we provide. Once we receive it, we’ll schedule an assessment, and you’ll arrange the physical and TB screening with your doctor. When everything is ready, we’ll set your loved one’s start date and warmly welcome them to our program.

Get Started
1. Tour and Introduction
We begin with a personal tour of our day center. During your visit, you’ll have a chance to meet our staff, see our program in action, and ask any questions you may have. This gives you a clear picture of what we offer and helps us start getting to know you and your loved one.
2. Application
After the tour, we’ll provide you with an application packet to fill out. Please complete all sections and include the application fee. 3. Assessment, Physical, and TB Screening
When we receive your application and fee, we’ll schedule an intake assessment. You’ll meet with one of our social workers, our nurse, and our program director, along with your loved one and any family members who wish to attend. During this visit, your loved one will spend some time joining our activities so we can learn more about their needs and the best level of care. You’ll also complete the enrollment forms and talk through transportation, medication, or dietary needs. Before starting at the center, your loved one will need a physical and TB screening completed by a doctor using Insight’s required forms. These forms must be signed and dated within 30 days of the planned start date. Our nurses can help prepare the paperwork to make sure it meets all licensing requirements, and if needed, we can connect you with a physician we work with for an additional fee.
4. Start Date – Chosen by Us, Just for You!
Once the physical paperwork is completed and submitted, it’s time to join us! Your loved one is ready to have fun with us at the day center!
